* Social Security Card

* Birth Certificate

*Current Alabama Immunization (Blue Form)

*Proof Of Residency (Two Forms)

*Withdrawal Form/Grades from Previous School

*Transcript from Previous School

Withdrawal Procedures

In order to withdraw from Dothan High School the following must be done:

*All textbooks returned to teacher or attendance clerk

* All uniforms should be returned to ROTC, Band, Choir, etc.

*All fees and fines must be paid

*Transfer information must be given to guidance

*Exit counseling interview with counselor/administrator
